lug studs

how easy / difficult is it to replace the rear studs on a 91 z28 with drums in the rear?

Bang them out with a hammer, bang the new ones back in. A serious rusting issue might require removing the axle and taking them to a press to press them out, but I've never heard of anyone having to do it. Hose them with PB blaster a day before several times, then beat them with a short-handled 5lb sledge.
