A Friend of mine made new ones from copper pipe and sweated them together. Look around and you should be able to find some sweat fit hose barbs for the ends.

Looking at the picture, it appears that there is just a single hose going to the vacuum canister. Did you just place a "T" in the line from the plenum to the brake booster?
