Code 33 / Big cam????

Car: 1993 Z-71 pickup
Engine: Heavily modified 355 on TBI
Transmission: Had a 4L60e. Now a 700R4
Axle/Gears: Bone Stock
Code 33 / Big cam????
I'm at a total loss of how to correct this problem. It idles fine but after about 15 to 20 seconds after I put it in drive it stalls giving a code 33 / high map error.
I've driven it 2 years this way shifting it into neutral at every stop light because I cant figure out how to fix it.
16168625 ECM running E6 programming with Cats Tuner and logging with Datamaster.
1993 Chevy truck. Mid 70's 350Ci .030 over. Gear Drive, mild heads, Large cam but dont have the specs right now. The entire induction from the original 5.7 was bolted to the new block. It idles around 12 inches of vacume.
I've done as much as I can through the programming (such as changing the constant for the actual additional displacement) and I've got the fuel and spark tables pretty close. I just cant figure out how to keep the thing from dying several secoonds after putting it in gear. I know it has something to do with the cam / vacume but I dont know what to change to fix it.
The one thing I had to do to get a decent idle several years ago was to open the throttle blades a little (piece of paper as a feeler guage) then slotted the sensor to get the proper voltage afterwards. It didn't change the stalling issue.
Please help.

What is the noise from the gear drive doing to the knock sensor? Why would you install a gear drive?
How did you get the original intake manifold to bolt to the mid-'70's heads?

2) Personal preferance.
3) Easy. Machined metal pads on the intake to compensate for head / intake geometry.
