Loose (side-to-side) Brake pedal

Loose (side-to-side) Brake pedal
Well, brake AND clutch pedal.
I have about 1/2" play side to side in my brake pedal and a little less than that on the clutch. I tried to tighten the bolt that goes thru the braket and pedals which helped a little bit. The bolt measures .388 just below the sholder, then narrows to .356 1/4" down the shaft. It continues to narrow to .352 at the other end before the threads. The ID of the metal pins (for lack of a better word are about .426-.435 on one and .441-.446 on the other (best I could measure with a dial caliper)

Is everything just worn out or am I missing something like a sleeve? Or is my thinking completely wrong and I just need to really crank down on the nut and squeeze the bracket to hold the pins?
It's not a sudden occurance. Bought the car, drove it for a few years, then it was COMPLETELY torn down for about a year. Put it all back together, noticed the issue but couldn't remember if it was always like that or not. I drove it like this last year and it wasnt' until a friend drove it that he commented his camaro isn't like that.
