I assume the switch is bad? The fan works, but only when I turn the A/C on!

You could be right, but it might also just never be reaching the insane temp that the factory set your fan to come on. It's somewhere around 230 I think. Way too high in my opinion. But then I'm not a GM engineer...
There's a sensor under your passenger side exhaust manifold with one wire on it. Ground that and see if your fan comes on. If it does, then it's either that sensor or it just isn't reaching the temp to trigger it. (you can replace it with a lower one if you want) If it still doesn't come on, you have some other problem; burned wire, etc...
