Rear discs come right off after you remove the brake calipers, they're only held onto the axles by the wheel lugs.
Front discs are held onto the spindles by a large nut. Pry off the bearing cap (stamped steel cap right in the center of the rotor) and you'll see a big nut held in place by a cotter pin. Remove the pin, remove the big nut, and the rotor will pull right off...be careful not to damage the inner grease seal or bearings.
I hate to recommend Chilton's books but even these are better than nothing. You really don't want to be guessing your way around steering/suspension components.
