alternator woes

I'm on my second alternator and had to swap the first one out because of some clacking that increased with engine speed. At first I thought it was the air pump (L98) engine.

The clacking eventually turned into more of a grind/squeal and eventually my belt slipped off because the alternator "arbor" was pulled through the nut and the whole pully came off. There were no more threads left on the shaft (all stripped)!!

Any idea of what could be causing this? As I said it's happening again and it's a rather expensive hobby, replacing alternators.
