Emission control hose routing sticker

Does anyone know where I can get a copy of the emission control sticker for my car, preferable on the internet.

Classic Industries has them, but not on there site. If you call them @ 1-800-854-1280, they are $5. They will need to know if its a 305G, 305H, or 305F (LG4, L69, or LB9) Auto/manual trans, and Federal/California emissions.
