tach jumpy, car stalls??

Okay guys, running into an issue. But trying to diagnose it but having trouble.

Car runs perfect on a cold start up or after its sat for a few hours.

After the car warms up, you can visibly see the tach jump anywhere from 100-1000. The car will also jerk when the tack does this. Sometimes it will even stall out, but will stop stalling if you engage the clutch?

cant seem to figure it out, I'm thinking maybe vacuum issue, fuel pump, or spark plugs?? Any other ideas??

1983 2.8L 5 speed.
