Need help on cam ID

Car: chevy 1500
Engine: supercharged 350 sbc
Transmission: NV3500
Axle/Gears: 14 bolt 5 lug 3.73 gears
Need help on cam ID

Looking for some help with id on the cam i pulled from my engine, 630 cpc bc 5 is the numbers i found off the cam, the block casting number is 14093638 2 bolt main and it had 10088113 aluminum heads, crank numbers are 14088526. Also main caps have FM on them and i was told that means they are forged not sure if thats true?
