The posi is worn out. In the case of a 9 bolt, the clutch cones and inner housing is worn.
I am not allowed to post a link or give you information on this, so hopefully someone else will!

Thanks ede!
The cones ride against the inside of the carrier halves. As the cones wear, they ride deeper into the carrier. Once they bottom out, then the posi stops working and the cones start chewing up the inside of the carrier halves. The only way to know for sure is to take it apart.

If this is any interest, I saw a 9-bolt on a '85 T/A in the boneyard yesterday. No one had even messed with it yet. Tires were still on the rear-end. Still had the tag on the back. If it is still there tomorrow, I will pull the cover and take a look at the gears.

alright so it should be visible, would this wearing of the carrier cause a "clunk" or somethin, i have two nine bolts that clunk going into gear.

The other will clunk and spin both tires on wet pavemant AND dry pavement.

correct me if I'm wrong, but from what I've been told, most of the time, true posi rearends make a clunk. don't know why, they just do, prob something to do with the posi unit.

No
A posi unit should not clunk.
The clunking noise is coming from excessive back lash, or play in something. If the posi doesnt' work, the cone clutches are worn out/and/or the springs are weak. In my expirience, it's usually both. It's as simple as that.
