T56 pops out of gear

I've been having a problem lately with my T56 in my 99Z28 popping out of 2nd and 4th gear and i can't get them to engage. I am thinking it might be a shifter problem because you can see and feel that its not going all the way in the gate.
I have a B&M ripper, I am using the stop screws (the rear one might be too far in or something) and i used the washers to offset it towards the driver. I plan on doing a reinstall soon using new bolts, a new bushing, and the standard washers. What could be causing a problem like this?

I'm interested in this as well, my shifter pops out of 3rd gear ALL the time.. at least a few times on a say 15-20 minute city drive.. It happens right after I put it in gear then let off the clutch and its gets more annoying every day.. I'm just afraid my first time to the track with the 6 speed I'm going to end up poping out of 3rd and revving real high and ruining my engine.. I also have a problem where I can't downshift 6th to 5th, which isn't really a big deal beacuse I don't really hit 6th that often.. I am assuming its just the synchros for that problem..

My T5 does this in first gear and I have been told it may be worn shift forks or bad synchronyzers.

I wouldn't use the Shifter stop bolts at all, they aren't needed on the T56. The T56 has its own built-in shifter stops. Now on the 3rd gear popping out on Decel, there is a TSB on that. I've been replacing the 3-4 shift fork, 3-4 syncronizer, 3+4 blocking rings. At the same time I usually do the same things for 1-2 gears if it seems necessary.

Joe, you got it right... i pulled the console and boot tonight and i found the rear stop bolt was keeping me from engaging 2nd 4th and 6th gear... removed them, went for a test drive. Now if i could just get it to be quieter...
