How did this happen?
I was driving my car and when I started it was fine. then whenever I pushed the clutch in to shift it made a ticking sound (I think the clutch fork was hitting the throwout bearing) and in 3 minutes of driving it went from light noist to the engine being stopped by the clutch pedal trying to push it far enough to shift. I took it all apart today and the throwout bearing was completely bent. If I replace the throwout bearing and put it all back together will everything be good, or is there another problem that caused this?

No doubt that the throwout bearing is destroyed. The question is: WHY? It looks like the typical case of the irresistible force encountering the immovable object.
How many miles has this setup been installed? For the price of a new clutch, I think I would replace the entire clutch assembly, especially if it's been in service for a while, and go over the whole thing with a fine-toothed comb while I had it apart. I'd want to know what caused this, and it doesn't look much like metal fatigue to me.
How does your input shaft on your trans look? I wonder if the throwout bearing could have hung up on it somehow.

I've had it in for about 1500 miles. I put it in when I put the t56 in my car. The input shaft looks fine. When I took the slave cylinder out the clutch fork wasn't even all the way on the throwout bearing, and I wasn't able to get it on it.

Maybe that was your problem. I understand that the T56 pulls to release rather than pushing... so it would seem that either your clutch did not want to release, or your fork was placing uneven loads on the throwout bearing.
At this point, I think I would get a replacement throwout bearing, and make sure it moves properly on the front collar on your transmission without binding. Then install it in the car with a clutch pilot tool in place. Then I'd have someone depress the clutch pedal while you watch what happens.
If you have difficulty installing the fork on the bearing, you should then troubleshoot that until you resolve it. If it all looks good, you might as well put it all back together and see what happens.
