Check the universal joints on the propshaft too (driveshaft in USA?) - if one or both of those has worn then it will cause severe vibration at relatively high speed.

FWIW are you blipping the throttle on the down change (you know, rev the motor slightly), to help the synchromesh a little?

I know this won't solve the problem but it may lessen the grinding.
