Exhaust noise comes from what?
Just wondering how exactly aftermarket exhausts increase a cars loudness.Is it the actual muffler that just flows more or is it the bigger pipe?

The larger piping is the biggest factor when making exhaust louder. Muffler flow and design is another factor. Aftermarket "performance" mufflers do not dampen the sound as much as stockers and therefore typically flow more. There is far more to it than this, but this is just a basic run down.
