I have Headman LT D port headers #68469 same tube diameter and length as yours. I used Pacesetters LT1 slip fit Y pipe for LT headers #821161($120.) From the headers I used 3" slip fit collectors to the y pipe. I had to fab a bracket to support the y pipe and put a small s bend to clear the torque arm. I then used a gutted cat(have to pass visual inspection) and 4th gen Magnaflow cat back both bolted up perfectly. Out back just had to tinker with the exhaust hangers to get them to fit. It all worked out pretty well and I didn't have to pay a muffler shop the $300 they wanted for a custom y pipe.
