Need help identify this label

Had it on my GTA but removed it long time ago. Need to reproduce it, would appreciate if someone could take a good picture for me or guide me what to search for. (can't read the text)

Here's what I think it says. The last line wasn't visible in the photo so I made up what it I thought it might say. I estimated the proportions, so not sure that its exactly right. Same with color

In the past I've worked through a local sign shop on a few small projects. I supplied an image file and dimensions and they were able to print on 3m vinyl and then apply some sort of clear layer to weather proof. Probably not the best way, but works okay.

Thanks! I think I need a photo of the original so I can match font more exact. I have reproduced some labels myself by printing on vinyl and apply clear on those who should have it. I work as a designer and have access to printing tools. On this one, if I remember the original correctly, it is printed on thin matte paper without coating.

You can get some sticker material from onlinelabels.com and using a Laser printer make one that looks pretty close...

Here's the result. Compared fonts and found one close to original that I modified to match. Then I made the graphic in illustrator as vector, laser printed on semi-matte label paper and cut the outlines on a vinyl cutter. The photo doesn't show true blue color tone.
