Mysterious 6 month problem HELP

Hey all,

Ive had this problem as long as i've had the car.

Start her up, everything is good. Purrs like it should. However, after about 3 minutes there is an EXTREMELY definite change in engine sound... as I type this I'm deciding to make a video... (will post in here when added)

The idle will decrease, and then decrease more to the point where its fluctuating above dying, and then just idles back up and repeats the process.

I have: Replaced egr, cleaned iac, cleaned maf, no error codes or faults.
The fan is hardwired to always run (Thank the previous owner for that, shade tree was his name i believe) so could it be that the engine is just not reaching optimal running temp?

UPDATE: I think its worth mentioning that im still getting codes 32 33(?) and 36.

I HAVE REPLACED THE EGR, it should not be throwing code 32 and im 90% sure the MAF is ok because there is no idle change when it is unplugged.

No, but I dont see how that could be an issue if it idles fine for a while then does this sputtering. Not timing. All injectors are firing at the same rate.

Wouldn't the computer change the timing after the engine warms up?

Yea, but it wouldn't be sputtering and going up and down if it was a timing issue. I mean obviously the timing is off but its due to other factors.

I was getting "Exhaust rich" codes for a while however the ecm is no longer throwing them. I also did a voltage test on the o2 sensor and determined it is outputting a stable and correct voltage.

Also, the cycling on my readouts (not on the charts i posted here, sorry) lead me to believe its functioning properly as well.
