Seems too rich at startup, you need to hook a scantool into it, and watch the startup parameters, CTS,...might be a simple sensor problem, or coldstart injector, MAF values, bad injector(s), etc....that's why a quick look at at what the ECM see's at startup, can point you in the correct direction.

Black-running rich
Blue tint-burning oil
white-burning water/coolant

Are you absolutly positive its white after the puff of black and not really a light tint of blue? What does it smell like (if you can get a smell over the puff of too much fuel)?

I've been a transport refrigeration mechanic for over 30 years working on small diesel engines, and I've never seen the difference between "white smoke" and "bluish smoke", I would bet he is seeing a puff of "blue" smoke and probably has some valve guide seals leaking. Undoubtedly, he has a fuel rich problem on cold start. By the way, does it put out black smoke on start up every time, or just when cold?

I vote for the valve seals.
