Excessive valve train noise?

Excessive valve train noise?

The passenger side valve train keeps making a loud clacking noise and I don't think its from loose rocker arms although it sounds like one is loose. Its loud enough to set off the knock sensor at certain loads and rpms, usually pretty light load.
Sounds like its the rear exhaust valve.
I don't think its the rocker arm adjustment being too lose as I've tried adjusting with 1/4, 1/2, 3/4 and 1 full turn on the nut for lifter pre-load after the lifter just begins to push down, and it makes the same noise no matter what.
I've even switched from roller bearing/tip rockers to plain stamped steel ones. and still the same noise!
The driver's side is fine.

The nut is not backing off as I marked it. Its pumping oil up thru the pushrod. Could the lifter not be holding enough pressure or something?
What else could it be??

Could be a bad lifter. It could also be a worn out rocker arm/pivot ball/stud. Could even be a really bad exhaust leak.

when you adjust the valves and shut engine off..
observe the rocker arms after shut down. ive
be seeing a lot of bleed off (valves in open
postition close) even had new lifter clatter
because check ball not seating holding pressure.
some of the lesser expensive lifters have had
this similar problem. selaed power lifters
(johnson lifters) dont seem to have this bleed
off. just an obseravtion...
