9 Bolt Spider gear cross shaft broke?

Any idea what causes this? Is this a common failure in the 9 bolts?
Its seems like the only way this could happen was if the spider gear sized on the shaft then it sheered off. The other part of the shaft is still jammed in the spider gear.

Seemed like the diff was working fine, only once or twice it did a one wheel burnout in the burnout box at the track. It made no noise so I didn't expect to find anything broken and thought it just needed to be shimmed.

The metal remains found their way between the side gears & posi cones and between the posi cones and carrier housing so everything is all scored up now.

Here are some pics

Well, you said it, the smaller pinions got some kind of contamination in them and the shaft seized. The rest is history.
