Both. The Haynes 82-92 Manual shows both ways. I changed mine when I replaced my timing chain, and I did it the easier way- with the cover off the engine. Doing it with the cover on the engine needs a special tool (or some tricky work).

I belive you have to take it off and remove it from the front so it comes out the back side of the cover
