Turns out the shifting problem was due to the gear on the shift governor being broken. Replaced the gear, shifts through all forward gears now. Thanks for your help!

The hold down bolt is tight. So are the two screws that hold the cap down. The distributor itself is not rotating, just the cap. And the metal plate the cap is screwed to.
