From what I've heard, he had taken apart the pressure relief blowdown valve, and added several rupture discs to prevent possible premature venting. This disabled the blowdown rupture valve, permitting the bottle to exceed its pressure capacity and explode - nothing to do with the safety of nitrous, just an individual user's stupidity.

Yeah I've seen that one before. What a mess!
I always wondered about it. Sometimes people accidently put in two rupture discs in the blowdown tube instead off one. They are thin and can stick together so it is easy to do. Also if it is bent or creased that can strengthen it. I wonder if any of these things are what caused it not to blow off.
I also wondered what caused the bottle to build that much pressure. I would assume it was heated by the bottle heater. Failed relay could cause it to stay on. I use two relays in series for redundant protection. If one fails and closes the contacts the other will still stop current flow. I also put a plug in the wires at bottle heaters and only have them pluges together when at the track or when planning to use the nitrous. When I open the bottle valve I plug the wires together and when I shut the valve I unplug them. I might be a bit paranoid but I don't want my ride peeled open like a Dole banana. LOL
Also for a little more peace of mind, I use fusible links at the battery for my bottle heater and solenoid supply wires. That way if something shorts I don't have a glowing wire catching something on fire.

As the story (rumor) goes on, it was the bottle heater that built up the pressure to begin with. He had supposedly wired the heater to a CONSTANT 12V source, instead of a switched ignition source. Another stupid mistake. I think I read that he accidentally hit the switch while cleaning his car, or getting something out of it. His supposed 3rd mistake? You would have thought that the thermostat would have done its job and turned off at 85 degrees, right? It would have...if he hadnt removed it.

Like said before...all rumors.
