SOLVE THIS ONE!!
This has been happening for 2 years now. I would start my car and it would rumble like if It had a huge cam. Then after about 2-3 seconds the rpms would finally sit where they are supposed to and the car would run and sound fine. And sometimes, the car would make the "big cam" noise and then shut off completley. Then I would have to turn the key 3-4 times to get the car running. All my ignition components have been changed, alternator and battery are brand new. Fuel pump relay? Fuel pressure regulator?

Does it puff a bit of blue on startup?
possibly getting oil in the cyls and fouling the plugs for a bit till it burns out.

^^ could also be a sticky IAC...
But that depends if the smoke could was actually blue (oil) or black (too much gas)
If its blue your valve seals are most likely the culprit unless it also smokes when its warmed up which would be rings.
