Not sure on the richness, but the idle and surging problems could be related to the IAC valve and it's passage in the TB. Pull it out and clean it and it's passage thoroughly and see if that helps the idle. Do you have any scantool data or DTCs from the car? A little more info on the application would help us help you as well.

Major Problems too!!!!
I have had similar problems with hesitation where the car jumps. Replaced fuel filter, vacuum hoses, MAF sensor, O2 sensor, spark plugs and wires, pcv valve, cleane3d out throttle body, put new intake manifold gaskets on too. It still kinda jumps at any speed when i slightly press the gas pedel. The injectors were cleaned also and i can't seem to fix the problem. If you find out let me know please!!

I also had the hi rev problem for a while they replaced the coolent sensor which stopped the veh from going into a closed loop after warm up and the iac valve which was malfuncting. Its been running pretty good since
