I don't think you're adjusting it right, or I'm having trouble understanding what you've been doing.
Press the button and slide the cable guide in until it is all the way in. Now let go of the button and floor the throttle. It is now adjusted. Anything other than this type of automatic adjustment will result in wierd shifting.

Follow the instructions I gave you and see if it improves the situation.

Not saying this is the case but might be a good idea to jack up the rear and check out the driveshaft yoke, make sure the caps and u-bolts are on snug and have no play, if its vibrating pretty bad, i'd have a look at that to make sure that isn't contributing to your problem.

The shuddering sounds like a torque converter that is failing to unlock. This happens, especially on higher mileage units. I would try unplugging the wiring harness from the trans and see if the problem persists.
Definately check your entire driveline for slop (pinion, yokes, u-joints, extension housing bushings). Go over the suspension too.
