700 clutch problems, more than once

Engine: LO3, 305 TBI Mildly Modified
Transmission: BakerBuilt 700R4 w/B&M Megashifter
Axle/Gears: 3.23 Auburn Pro Series LSD
700 clutch problems, more than once
i have had 2 3/4 clutch packs go out in 2 years on a built trans
its got a custom shift kit
line bias is blocked open it has full pressure all the time
2nd failure had an alto clutch pack
got the 4th at wot
custom servo
custom modulator
each one has lasted 1 year and about 8k or less miles
i have a real good trans builder and he is stumped on why they keep going out
any ideas or anyone have problems like this and did anything to fix it
