I'd say its your O2 sensor, when my O2 sensor went bad the car would run fine cold until it warmed up. While the EGR valve made it run poorly until warmed up.

YOur best bet, is take it to a local Garage and have them put it on an Analyzer. The Garage near me does it for $25, then you know exactly whats wrong.

I recently had this happen, due to someone else working on my car.
Mine is a new engine. I had 20 inches of vacuum at idle. Then I picked it up.
Knew something didn't wound right.
Played around and got it to run smooth, real smooth.
Why?
Vacuum leak at teh base of the distributor, intake manifold part to the PCV valve assembly.
My rubber hose was cracked. So I took a zip tie to the worst part and that cured it. Engine now reading at 19 inches again.
I think you should try all hoses. Bet, you'll find it there. 99% can be fixed with 5/16" windshield wiper hose and new T fittings are available. NONE of the hard plastic hoses are available. ALL discontinued.

Listen for the smoother running engine when ya find the leak.
That's how ya tell.
And a vacuum gauge is a very valuable tool for your car. Should invest the few dollars for one.

You just dive in and follow the hoses. Look at all juctions, underside of the hoses.
You ma find the problem, real easy.
Again, just listen for the engine to run smoother. That'll let ya know you found a weak spot.
